Continuity of care from the hospital: interdisciplinarity and devices for integrality in health care networks

Abstract

The aim of this study is to analyze a scientific production on the continuity of care and comprehensiveness of care, identifying the devices and tools used in daily work for this construction from the perspective of the hospital. We opted for an integrative literature review carried out from june to july 2021. Searches for articles were carried out in the PUBMED / Medline, Lilacs, Bdenf, and Lis databases. The exclusion and exclusion criteria, the final sample consisted of 36 articles. From the analysis of the studies, three categories emerged: Health care and integrality within the hospital; The link and as linked to the continuity of care; Health care networks in the Unified Health System and continuity of care. Analysis of the studies needed to identify some work organizations in the micropolitical dimension of health care production. The main goals and devices found were safe discharge, bed runs, clinic management, case discussion, lines of care, link nurse, regulators, permanent education and multidisciplinarity. It is concluded that there is a need for changes in work processes and in the organization of health care services in networks to guarantee the integrity and continuity of health care.
